APPOINTMENTS

For an appointment, please call 281-583 1300.
If you are a new patient you will need to fill in a new patient questionnaire before you come in to the office. You may download this form from our web site.
We will make every effort to see you within 24 hours if you need to be seen urgently. If you have any special requirements please let us know.

 

PREPARING FOR YOUR APPOINTMENT

· Relax! We will do everything we can to make your visit both comfortable and pleasant. Once you have booked an appointment, you may print patient information form online, fill it and bring it with you to your appointment. If you have a fax we can also fax a new patient questionnaire to you. Taking time to fill this form prior to your appointment saves you precious waiting time.
· If this is your first visit to us please try to arrive early - give yourself plenty of time to find our office. For your first visit you will need to fill out some forms. The information is then entered into our computer by the staff. This entire process can take a while so please try to come early. If you wish to save time you can even fax or mail your form to us so that your information is already entered into the computer when you arrive.
· Please bring your insurance card and other pertinent papers with you.
· You may also wish to bring in your checkbook or a credit or debit card for your co-payments.
· Call your referring physician's office to ensure that they have faxed in a referral and your medical history and reasons for referral to us. Please remember that we need as much medical information as possible from your referring physician to make your visit meaningful.
· Bring all your medicines with you.
· Bring someone you trust who can also listen while the doctor discusses the treatment options with you.
· Write down all the questions that you may want to ask the doctor. You will have ample time to ask the doctor all these questions after he has finished explaining everything to you.
